5Cs has a large community network of neighborhood centers, shelters, and outreach teams that can easily connect our neighbors facing housing instability to our services and support in finding safe and appropriate housing.

assistance to members who have secured a new housing unit to have a smooth and seemless transition to community living. includes set-up of new housing unit and environment, i.e. furniture, adaptive aids, food and clothing needs at transition.
tenancy sustaining services to assist in maintaining member tenancy in affordable or supportive housing by providing tenant rights education and eviction prevention.
